About The Position

Serenity Hospice PA is seeking a compassionate Licensed Practical Nurse (LPN) to provide skilled nursing care to patients in Dauphin, York, and Cumberland Counties, Pennsylvania. Requirements

Active Pennsylvania LPN license required
Valid driver’s license and reliable transportation

Nice To Haves

Previous hospice or home health experience preferred
Strong clinical and assessment skills

Ability to work independently while being an engaged member of a team

Responsibilities

Provide skilled, compassionate nursing care under the direction of an RN.
Help patients remain comfortable and maintain their dignity and quality of life.
Provide individualized, patient-centered care based on each patient’s care plan.
Administer medications and treatments as ordered.
Monitor patients and communicate changes to the RN and care team.
Help patients and families understand care needs, medications, and comfort measures.
Maintain accurate and timely clinical documentation.
Collaborate with the interdisciplinary hospice team.
Build trusting relationships with patients, families, and caregivers.
Be a compassionate and dependable presence when patients and families need it most.
