About The Position

Provide intermittent skilled nursing care in accordance with the established plan of care under the direction of the assigned primary nurse/supervisor. Gives direct client care which includes changing dressings and catheters, administering prescribed medication and treatments, giving necessary injections, enemas, ostomy care, and pain management. Educates the client’s family in terms of how best to cope with the illness and end of life care. Carries out nursing assessment and care based on referral data, client/family interview, and clinical observation of patient health status, symptom management for hospice and palliative care patients. Records daily progress on charts of clients visited. Reports to primary nurse, supervisor, physician, social worker, or therapist regarding cases. Consults with physician regarding client needs and plan of care. Assists in evaluation of care given by paraprofessional staff. Participates in Team meetings and case conferences with Hospice interdisciplinary team members. Seeks consultation and guidance from supervisor. Maintains productivity standard. Provides continuous care for hospice patients as needed. Will actively participate in the performance improvement plan known as H3W. Performs other duties as assigned. Coordinates agency service delivery and solves logistical problems for customers and clients. Provides client information to physician’s offices and receives care plan changes via verbal orders. Documents specific patient information including verbal orders from physicians; transports verbal orders from physician’s offices as needed. Intervenes via telephone and/or through the application of tele-home care technology for patients; communicates changes in health status of patients to appropriate clinicians. Refers callers to appropriate community resources or agency services when needed. Receives and relays information between field staff, physicians, and other resources.

Requirements

  • Licensed practical nurse with a license to practice in the state of CT
