A Typical Day for a LPN Visit Nurse with WesleyLife at Home Hospice: Your day begins with intention — grounded in WesleyLife’s belief that every person deserves comfort, dignity, and the opportunity to pursue joy, no matter where they are in life’s journey. As a Hospice LPN Visit Nurse, you’ll spend your day providing gentle, compassionate care in the homes of those we serve. Each visit is unique because every individual’s story, needs, and wishes guide your care. You’ll offer personal support with daily activities, provide comfort-focused care, and be a reassuring presence for clients and their families.

Requirements

  • Current Openings: PRN (Part-time as needed)
  • Perfect for LPN's seeking a flexible schedule with meaningful work.
  • Current LPN license
  • Successful completion of a pre-hire health assessment
  • Successful completion of a drug screen

Responsibilities

  • Travel to clients’ homes and care settings to provide comfort‑focused, person‑centered support to those nearing the end of life.
  • Deliver hands‑on nursing care, symptom relief, and comfort measures.
  • Assist with daily activities and ensure clients’ physical and emotional needs are met.
  • Monitor changes in condition and communicate with the interdisciplinary team.
  • Support families through education, reassurance, and compassionate presence.
  • Work independently in the field while collaborating closely with RN Case Managers, social workers, aides, and spiritual care professionals.
