About The Position

Join Our Hospice Team We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Hospice team serving patients in the Tiffin and Fremont communities. This role provides an opportunity to make a meaningful difference by delivering high-quality nursing care and support to patients and families during their hospice journey. As a Hospice LPN, you will work collaboratively with an interdisciplinary care team to ensure comfort, dignity, and exceptional patient-centered care.

Requirements

Current Ohio Licensed Practical Nurse (LPN) license in good standing.
Graduate of an accredited practical nursing program.
Previous hospice, home health, long-term care, or medical-surgical experience preferred.
Strong assessment, communication, and documentation skills.
Ability to work independently while maintaining excellent teamwork.
Valid driver's license, reliable transportation, and acceptable driving record.

Nice To Haves

Hospice or home health experience.
Experience working with electronic medical records.
Knowledge of end-of-life and palliative care principles.

Responsibilities

Provide skilled nursing care to hospice patients in accordance with the plan of care.
Monitor patient conditions and report changes to the RN Case Manager and clinical team.
Administer medications and treatments as ordered.
Document patient care accurately and timely within the electronic medical record.
Educate patients and family members regarding disease processes, medications, and comfort measures.
Collaborate with physicians, nurses, social workers, aides, and other members of the hospice team.
Support patients and families with compassion, empathy, and professionalism.
Participate in care conferences and interdisciplinary team meetings.
