Requirements

• Hold a Licensed Vocational Nurse (LVN) license in good standing with the state board of nursing.
• Have a minimum of 2–3 years of hospice experience.
• Strong communication skills and the ability to work collaboratively with healthcare professionals.
• Proficiency in electronic medical records (EMR) systems.
• Excellent attention to detail, critical-thinking, and problem-solving skills.

Responsibilities

• Administer medications, immunizations, and treatments as prescribed by physicians or other healthcare providers.
• Monitor and document patients’ vital signs and medical history.
• Develop and maintain patient care plans with input from physicians and other healthcare professionals.
• Educate patients and their families about health conditions, medications, and treatment options.
• Collaborate with physicians, nurses, and other healthcare professionals to develop and implement patient care plans.
• Follow proper infection control procedures.
• Document patient care and progress in electronic medical records (EMR).
