About The Position

This part-time Hospice Licensed Practical Nurse role involves providing skilled nursing care to patients at the end of life, conducting visits in facilities and/or at home. The position also includes providing on-call services after hours and completing skilled visits. The LPN will assist Registered Nurses in carrying out the plan of care, including performing specialized procedures as directed by the RN and under physician's orders. Key responsibilities include actively participating in interdisciplinary care planning to ensure the patient plan of care is current and issues are addressed, reinforcing patient and family education regarding end-of-life care, and keeping the RN Case Manager informed of any changes noted during patient interactions. The role also emphasizes personal and vocational growth through participation in nursing organizations, in-service programs, further education, workshops, conferences, and individual research.

Requirements

  • Licensed Practical Nursing Degree
  • A minimum of 1 year of Clinical experience
  • Valid driver's license and Auto insurance Coverage
  • Reliable transportation: Willing to travel

Responsibilities

  • Provide skilled nursing care to patients in EOL, conducting visits in a facility and/or home patients.
  • Provide on-call services after hours, and complete skilled visits.
  • Assist Registered nurses in carrying out the plan of care.
  • Assist RN in performing specialized procedures (as defined in the state nurse practice act) as directed by the RN under physician's orders.
  • Actively participate in the IDGH in collaborating to ensure the patient plan of care is kept current and identified issues are addressed.
  • Assist in the reinforcement of patient and family education regarding end-of-life care.
  • Assume responsibility for personal and vocational growth and development through active participation in nursing organizations and participating in in-service programs and other on-the-job learning.
  • Keep the RNCM informed of changes noted during the process.
  • Participate in self-development by practicing further education, workshops, and conferences, active in professional and related organizational meetings, and individual research reading.
