About The Position

Under the supervision of the Director of Hospice Services, the Licensed Practical Nurse (LPN) provides day-to-day nursing care to hospice clients. The role includes supporting clients with bathing, dressing, eating, toileting, and daily exercise; changing bandages, dressings, and catheters; and monitoring and assessing client condition through vital signs and other observations. The LPN plans and manages care according to each individual's needs and explains care clearly to clients and their families.

The LPN provides skilled nursing care under the supervision of a Registered Nurse, implements individualized plans of care, administers authorized medications and treatments, reinforces patient and caregiver education, and supports pain and symptom management. The position also involves clinical documentation, coordination with the interdisciplinary group, emotional support for patients and families during end-of-life care, and assigned on-call duties.

Requirements

• High school graduation and graduation from an accredited LPN school.
• Current LPN licensure in Oklahoma.
• Current Basic Cardiac Life Support (BCLS) certification.
• Current driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation.
• Active car insurance.
• Medical knowledge, patience, compassion, kindness, critical-thinking skills, knowledge of infection-control practices, stamina and physical endurance, observational skills, knowledge of occupational hazards and safety rules, communication skills, leadership, and empathy.
• Ability to perform extensive driving and assist clients with transfers and ambulation.
• Ability to occasionally lift and/or move over 100 pounds.

Nice To Haves

• Current Advanced Cardiac Life Support (ACLS) certification.
• Two to three years of recent hospice LPN experience.

Responsibilities

• Provide skilled nursing care to hospice patients under the supervision of a Registered Nurse.
• Implement the individualized plan of care established by the RN Case Manager and interdisciplinary group.
• Record patients’ histories and vital signs, including blood pressure, temperature, and weight.
• Observe, report, and document patient condition, symptom changes, and responses to treatment.
• Administer medications and treatments as ordered by the physician and permitted by Oklahoma law and agency policy.
• Reinforce patient and caregiver education regarding medications, disease processes, safety measures, infection control, and comfort measures.
• Assist with pain and symptom management interventions.
• Provide Foley catheter care, wound care, ostomy care, and other skilled nursing procedures within scope of practice.
• Promptly report significant findings, changes in patient condition, or safety concerns to the RN Case Manager and physician as appropriate.
• Complete timely and accurate clinical documentation in the electronic medical record.
• Participate in interdisciplinary group meetings and care coordination activities as assigned.
• Support patients and families emotionally and professionally during end-of-life care.
• Maintain patient confidentiality and comply with HIPAA requirements.
• Participate in agency prevention and safety standards.
• Perform on-call duties as assigned.
• Perform other duties as assigned.
• Assist clients with bathing, dressing, eating, toileting, exercise, and transfers as needed.
• Change bandages, dressings, and catheters.
