About The Position

This is a full-time position in the Kingfisher area. The shift is FRIDAY 5:00 PM - MONDAY 8:00 AM. Travel is up to 100%. The role involves overseeing patient cases from admission to death/discharge, completing nursing assessments, maintaining medication profiles, updating the plan of care, and participating in the re-certification process. RNs will conduct supervisory visits and admissions, while LPNs will assist in patient care within the established plan. Both roles require providing direct patient care, monitoring medications, taking vital signs, reporting changes in patient condition, and communicating with the interdisciplinary team. The position also involves patient and family teaching, adapting to workload changes, and performing duties independently, including after-hours calls. Participation in knowledge sharing through in-services and staff meetings is expected.

Requirements

  • Current Registered Nurse license (for RN position).
  • Current CPR Certification (for RN and LPN positions).
  • High School Diploma, or GED equivalent (for LPN position).
  • Licensed Practical Nurse Certification (for LPN position).
  • Excellent verbal and written communication skills.
  • Excellent interpersonal and customer service skills.
  • Excellent organizational skills and attention to detail.
  • Excellent time management skills, with a proven ability to meet deadlines.

Nice To Haves

  • 1-2 years of experience in home health or hospice, LTCF preferred (for RN position).
  • 1 year of experience in home health, LTCF, or related field nursing preferred (for LPN position).

Responsibilities

  • Oversees patient cases from admission to death/discharge.
  • Completes all nursing assessments for each patient assigned.
  • Responsible for maintaining patient medication profiles, updating as needed, while ensuring updated profile is in patient charts at the office, as well as in the facility or home.
  • Updates the plan of care for each assigned patient every two weeks.
  • Completes the re-certification process by assessing and accurately documenting findings on appropriate forms.
  • Makes RN supervisory visits every 14 days on all assigned patients.
  • Completes admissions as assigned.
  • Provides on-call hours as determined by Patient Care Manager.
  • Accurately communicates patients’ information to Patient Care Manager and medical staff.
  • Participates in sharing of knowledge through in-services, staff meetings, and assisting in orientation of new personnel.
  • Performs routine duties without physical supervision.
  • Recognizes and responds to priorities in the patient care process.
  • Makes sound judgements based on factual information, while seeking guidance and direction as needed for performance of all duties.
  • Provides support to patients and families in a courteous manner with a positive tone.
  • Assesses, plans, implements, and evaluates patient care.
  • Provides nursing care to patients according to established policies and procedures.
  • Checks status of medications provided by Hospice at each visit, and reorders as needed.
  • Takes and records vital signs of the patient in an accurate and precise manner.
  • Reports and records any changes in condition or behavior of the patient.
  • Communicates concerns and/or changes to all disciplines.
  • Assists in the assessment, planning, implementation, and evaluation of patient care within the nursing care plan.
  • Uses proper techniques, procedures, and safety precautions to meet individual needs of patients.
  • As authorized, prepares for set-up, hangs, monitors, and discontinues intravenous fluids with accuracy according to direction.
  • Provides direct care to patients, ensuring safety and comfort of the patient.
  • Performs treatments and/or assists in treatments and procedures with attention to detail, accuracy, patient safety, and comfort.
  • Records all care provided, following established guidelines for charting.
  • Utilizes the nursing process, and is familiar with other duties, recognizing their importance to patient care.
  • Participates in patient and family teaching and instruction.
  • Responsible for completion of assignments while maintaining quality work.
  • Able to adapt to changes in workload or patient assignments without interference to patient care.
  • Able to perform in an accurate and precise manner in emergency situations when time is of the essence.
  • Establishes good rapport with patients, families, and visitors, respecting patients rights to privacy and confidentiality of information.
  • Communicates pertinent information regarding patient to Interdisciplinary Team, using nursing care plan approach.
  • Independently performs assigned duties, including after-hours calls.
  • Assist in maintaining equipment assigned to patient care, and reports malfunctioning of equipment to appropriate personnel.
