About The Position

Provides in-home services to hospice patients, delivering skilled nursing care ordered by the physician according to the plan of care. This role involves assessing patient needs, educating patients and families, coordinating with the interdisciplinary team, managing pain and symptoms, and maintaining accurate electronic health records.

Requirements

  • Graduate of an accredited nursing program.
  • Current licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) in state of practice.
  • Two (2) years or equivalent full time experience in bereavement counseling within the last three (3) years.
  • Valid state driver’s license and reliable automobile, current automobile insurance, and be willing to operate personal car necessitated by nature of job.
  • Valid driver’s license and reliable transportation
  • Proof of automobile insurance (MVR checked)
  • Proof of current CPR certification
  • Able to pass criminal background check and national sex offender clearance

Nice To Haves

  • Master’s Degree in ministry, counseling, or social work preferred.
  • Clinical Pastoral Education (CPE) training preferred.
  • Experience as an LPN/LVN commensurate with one of the following preferred: One year experience in an acute care setting within the last 24 months; Six months home care experience within the last 12 months preferred.

Responsibilities

  • Provides skilled nursing care ordered by the physician as indicated by the plan of care to all patients on caseload.
  • Reviews caseload with supervising RN on a regular basis as established by best practice and/or State guidelines.
  • Obtains necessary physician orders and collaborates with the physician as the patient's needs dictate.
  • Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care.
  • Participates in coordination of hospice services, appropriately reporting the identified needs for hospice interdisciplinary group members (e.g. hospice aide, RN, OT, PT, MSW, ST, Dietitian, Chaplain or Hospice Manager).
  • Uses equipment and supplies effectively and efficiently.
  • Provides appropriate pain/symptom management. Evaluates patient’s response to treatments/medications.
  • Actively participates in Quality management program.
  • Completes accurate and thorough visit notes via Electronic Health Record (EHR) on every patient in a timely fashion.
