Requirements

  • Graduate of an accredited Licensed Practical Nurse (LPN) program
  • Active license to practice as LPN/LVN in the state in which hospice care is provided
  • Minimum one year of nursing experience

  • Hospice experience preferred

Responsibilities

  • Responsible for frequent and timely communication to the appropriate registered nurse, supervisor and/or other staff members regarding the patient’s condition. Communication includes participating in case conferences and other more immediate methods of communication when indicated.
  • Provides accurate and timely documentation of patient services to reflect the plan of care.
  • Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and self plan of care.
  • Administers medications and treatments as prescribed by the physician and in accordance with nursing standards of practice.
  • Participates in coordination of hospice services, appropriately reporting the identified needs for hospice interdisciplinary group members.
  • Ensures that equipment, supplies, drugs, biologicals and other necessary items and services are available.
  • Provides education and interventions to support the effective management of the patient’s pain and other physical symptoms as well as assesses the patient/family’s response to intervention and treatment.
