About The Position

Select Home Health & Hospice (A Fortis Health Company) is a clinician-led and operated skilled home health care agency serving central Indiana. At Select Home Health & Hospice, we believe in putting patients first: “Select the best for your health!” The Hospice Liaison will build and grow referral relationships and drive census growth. The Hospice Liaison will focus on educating the community about Select Hospice services while expanding the network of referral sources.

Requirements

• Valid driver’s license, auto insurance, reliable personal transportation, and ability to travel within the assigned territory (required).

Nice To Haves

• Bachelor’s degree in Business, Marketing, Healthcare Administration, or related field (preferred).
• Minimum 2 years of sales experience; home health or hospice experience (strongly preferred).
• Familiarity with CRM platforms (e.g., Trella Health’s Marketscape) is highly desirable.

Responsibilities

• Execute sales plans and track activity in CRM.
• Expand referral networks and monitor market trends.
• Build and maintain referral relationships.
• Serve as liaison with clinical teams.
• Represent Fortis Health at events and networking opportunities.
• Maintain accurate records and ensure regulatory compliance.
• Complete reports and support growth initiatives.
• Collaborate with managers and HR as needed.
