About The Position

The Hospice Care Consultant (HCC) role is a field-based position responsible for representing Moments Hospice to referral sources. The role involves building relationships, connecting with potential patients and families, and ensuring follow-through on referrals. Requirements

• Relationship building skills.
• Ability to represent something they believe in.
• Drive, empathy, and follow-through.
• Experience in hospice or home health sales.
• Experience as a healthcare account executive.
• Clinical background (e.g., RN, social work, chaplaincy) with a desire to move into sales.
• Experience in medical device or pharma sales.
• Experience as a clinical liaison or RN liaison.
• Experience in SNF or AL business development.
• Outside sales experience from other industries.
• Readiness to step into hospice sales with the right drive and heart.

Responsibilities

• Represent Moments Hospice to referral sources.
• Build real relationships with referral sources and potential patients/families.
• Connect with potential patients and families.
• Follow through on referrals.
• Collaborate with clinical and operations teams to ensure promised services are delivered.
