Hospice Liaison, RN
About The Position
Good Samaritan Hospice is looking for two full-time Hospice RN Liaisons to join their team. In this role, the RN Liaison will meet with patients and families who are considering hospice care, providing clinical assessments, answering questions, and helping them navigate an important decision with confidence and comfort. The liaison will also build strong relationships with hospitals, referral sources, and community providers to ensure a smooth and supportive transition into hospice services. This role is for a nurse who enjoys both clinical care and relationship-building. The position will also assist with admissions when needed and play a key role in ensuring patients receive compassionate, high-quality end-of-life care.
Requirements
- Current Registered Nurse (RN) license in the state of Pennsylvania
- Minimum of two [2] years nursing experience
Nice To Haves
- Marketing/Public Relations experience
- Admission and discharge planning experience
- Knowledge of hospice and palliative nursing
Responsibilities
