About The Position

As the Hospice Liaison and Admissions Registered Nurse (RN), you will serve as a key clinical representative and ambassador for the hospice program within hospital and skilled nursing facility settings. This role is responsible for providing timely hospice consultations, completing admissions, and strengthening referral partner relationships, with performance measured through clinical quality, responsiveness, and care transition outcomes.

Requirements

  • Graduate of an accredited school of nursing is required
  • Valid Connecticut Driver’s License and proof of current insurance coverage required
  • Current Registered Nurse (RN) license in the state of practice
  • Minimum of 7–10 years of clinical nursing experience
  • Strong assessment and communication skills
  • Ability to work independently in hospital and facility settings

Nice To Haves

  • Hospice or palliative care experience
  • Acute care, case management, or SNF experience
  • Knowledge of hospice eligibility and Medicare hospice regulations

Responsibilities

  • Provide hospice consultations in hospital and skilled nursing facility settings
  • Support appropriate and timely hospice referrals through clinical assessment and education
  • Serve as a clinical resource to physicians, case managers, social workers, and facility staff
  • Respond to consult requests within established service level expectations
  • Complete hospice admissions across assigned settings
  • Perform comprehensive eligibility assessments in compliance with Medicare and regulatory requirements
  • Educate patients and families on hospice philosophy, Hospice Medicare Benefit, services, and care transitions
  • Assist with coordination of care transitions for patients moving to another level of care
  • Ensure accurate, timely, and complete admission documentation
  • Maintain a consistent and professional presence within assigned hospitals and facilities
  • Build and sustain collaborative referral partner relationships
  • Model respectful, solution-focused, and patient-centered communication
  • Represent the hospice program with professionalism and clinical credibility
  • Collaborate with intake, case management, and hospice interdisciplinary teams
  • Identify barriers to timely admissions and recommend workflow improvements
  • Participate in quality improvement initiatives and program development
  • Ensure effective handoff communication to hospice care teams
  • Contribute to hospice census growth and program goals
