About The Position

Join Agrace, a nationally recognized nonprofit leader in hospice and palliative care. This is an opportunity for a skilled CNA who values meaningful, patient-centered care to make a difference. The Resource CNA Homecare (HC) enjoys variety by floating throughout Agrace's homecare service area to support different care teams and patients. They assist with activities of daily living, provide light homemaker support, follow the plan of care, communicate changes in condition, and document care while delivering compassionate, patient- and family-centered care consistent with Agrace's mission and standards.

Requirements

  • High school diploma or GED, or equivalent combination of education and work experience
  • Graduation from an accredited Certified Nursing Assistant program
  • Current, unrestricted Nursing Assistant Certification in state of employment
  • One-year personal care experience, hospice, or home health, preferred
  • Valid driver’s license, auto insurance, and access to a reliable vehicle
  • Basic Microsoft Office skills
  • Experience using electronic health records; Epic preferred
  • Ability to maintain confidentiality about patients, families, caregivers, staff, donors, and organizational intellectual property
  • Ability to use proper body mechanics, safe patient handling, and standard precautions

Nice To Haves

  • One-year personal care experience, hospice, or home health

Responsibilities

  • Assist Patients with Activities of Daily Living (ambulation, changing position, moving to/from sitting and standing, bathroom functions, bathing, showering, dressing, oral care, grooming, eating and drinking, post mortem care, obtaining vital signs, assisting with treatments, anticipating and responding to patient needs).
  • Provide homemaker assistance (light housekeeping, ordering and delivering patient supplies, helping prepare simple meals).
  • Communicate (read, understand, and follow the plan of care, document cares and changes in patient condition in electronic health record, document and report significant change in condition or unusual incidents, reinforce patient, family, and facility partner education).
  • Participate in meetings to plan and evaluate patient/family services.
  • Precept new staff and students.
  • Delegate tasks to volunteers as appropriate.
