About The Position

Concordia Hospice of Washington is a partnership between Concordia Community Support Services and Washington Health System that offers both quality home care and inpatient hospice end-of-life care. Concordia Hospice of Washington serves our patients in Washington and Greene counties with the focus to provide services that offer comfort, peace, and dignity for those with incurable illnesses and for their families. Hospice Aide Position: Dedicated and empathetic individuals are sought to join our team as Hospice Aides, providing personalized care to patients and their families during a sensitive time. This role offers the chance to connect with individuals, offer support, and bring dignity to their end-of-life journey. What you’ll do as a Hospice Home Health Aide HHA: Provide Personal Care: Assist patients with daily activities like bathing, grooming, dressing, and maintaining hygiene. Offer Emotional Support & Companionship: Build connections with patients and their families, providing comfort and a listening ear. Maintain a Safe & Comfortable Environment: ensure a clean and organized space, and prioritize patient safety. Monitor and Report Changes: Observe patient conditions and communicate any changes to the healthcare team. Support Care Plans: Work under the direction of a Registered Nurse to follow individualized care plans. Assist with Mobility & Transfers: Help patients move safely and comfortably, using appropriate techniques and equipment. About Concordia Hospice of Washington The roots of our organization can be traced back to Rev. Dr. J. E. Victor Carlson, who helped create Hospice Care, Inc. in Greene County in the late 1970s to serve families dealing with end-of-life illnesses. In 1996, the program was combined with The Washington Hospital, opening opportunities for inpatient care and other new services. On July 1, 2016, Concordia Community Support Services and Washington Health System (previously The Washington Hospital) partnered to form the organization we are today. Requirements

Certification: Current Certified Nursing Assistant (CNA) certification in good standing. (Recommended)
Experience: A minimum of six months of patient care experience, preferred in hospice, geriatric, or oncology. Training is available for the right candidate.
Qualities: A genuine passion for helping others, compassion, patience, and excellent communication skills.
Dependability: Punctual, professional, and reliable in following through with care plans.
Mobility: Valid driver's license, reliable transportation, and current auto insurance for travel to patient homes.
Skills: Ability to work independently and as part of a collaborative team, strong communication skills (written and verbal), and understanding of hospice philosophy.
