Requirements
- Current Registered Nurse (RN) license in the state of Michigan.
- Strong assessment, communication, and interpersonal skills.

- Valid driver’s license and reliable transportation.

Responsibilities
- Provide skilled nursing care to hospice and home care patients according to established plans.
- Assess patient condition and effectively communicate changes to the care team.
- Develop, implement, and update individualized care plans in collaboration with interdisciplinary teams.
- Educate patients and family members regarding illness, treatment options, and self-care techniques.
- Administer medications and treatments as prescribed by physicians.
- Document all patient interactions and care activities accurately and timely.
- Coordinate with physicians, social workers, and other healthcare professionals to provide holistic care.
