As a Hospice & Home Care Sales Liaison, you will impact the lives of our patients by – Analyzing the potential of the company’s success to determine target markets. Visiting doctor offices, hospitals, Assisted Living facilities, Skilled Nursing Facilities, and other possible sources of referrals to present agency credentials and obtain patient referrals. Analyzing the company’s organization to determine its strengths and weaknesses. Analyzing past and current marketing data. Analyzing patient/company relationships. Developing a marketing plan, identifying priorities and sets a reasonable timeline. Implementing marketing plan staying within established timeline. Reviewing and evaluating the analysis and plan on an established basis. Providing coordination of referral through all Dover Health services Attending IDG meeting, as appropriate. Serving as a first contact from clinical referral sources. Providing initial informational meetings with patients and/or families. Assisting office staff as needed with obtaining signatures and updates. Providing community outreach and education. Participating in discharge planning, as needed. Assessing patient’s/family members’ ability to cope with the patient’s dying. Functioning as consultant to the members of the health team, assists them in understanding the social, emotional, and environmental factors related to the patient’s health problems. Helping patients to utilize the resources to their families and the community.
