SCHEDULE: Monday - Thursday; 9:00am - 7:00pm; One rotating holiday per year LOCATION: Summa Health Akron Campus COMPANY: Summa Hospice HOW YOU'LL MAKE A DIFFERENCE: Care and compassion are at the heart of what you'll do as a Hospice, General InPatient Nurse-Case Manager. You'll help deliver care in a holistic, family-centered manner anticipating, preventing and treating undesirable symptoms or secondary symptoms. HOW YOU'LL WORK: You'll be responsible for providing end-of-life care and Hospice services to patients in the Health System Inpatient Hospice setting in a manner consistent with standards of practice. You will assist in determining the need to change the level of care based upon assessment and collaboration with the Attending Physician or Medical Director.

Requirements

Bachelor's Degree in Nursing preferred or Graduate of Diploma or Associate Degree program in Nursing within the state of employment.
Minimum 3 years inpatient clinical experience, preferably hospice.
Valid driver's license and auto insurance in your name as a driver.
Capable of all physical demands.

Responsibilities

Providing end-of-life care and Hospice services to patients in the Health System Inpatient Hospice setting in a manner consistent with standards of practice.
