About The Position

Tidewell Hospice, a member of Empath Health, provides compassionate hospice care to families across southwest Florida. The Hospice Field CNA works on the Home Team, covering Manatee and Sarasota counties, and provides personal care and household assistance to patients while supporting their primary caregivers. The role includes educating patients and caregivers about personal care, positioning, and daily living; following the written Hospice Aide Plan of Care; reporting unusual conditions or interactions to nursing staff; and working within the Hospice Aide scope of practice while protecting patient and family confidentiality. The schedule is Monday through Friday, 8 a.m. to 5 p.m.

Requirements

Florida State Certification as a Certified Nursing Assistant.
Current CPR certification.

One year of recent experience in hospice, home health, or an acute-care setting.

Responsibilities

Provide patient care, including taking vital signs, bathing, and household tasks that assist the primary caregiver.
Educate patients and primary caregivers about personal care, patient positioning, and daily living.
Support and carry out the organization's mission, vision, and values consistently.
Perform only the personal care and home maintenance activities included in a written assignment (Hospice Aide Plan of Care) by a health professional employee.
Recognize and report unusual patient conditions, incidents, behavior, or patient/family interactions to the patient's nurse or supervisory nurse.
Work within the Hospice Aide scope of practice at all times.
Maintain the utmost respect for patient and family confidentiality.
