About The Position

Advantage Hospice is a physician-owned, locally operated, patient-focused Medicare-certified and CHAP-accredited hospice agency serving patients and families throughout Northeast Ohio. We are growing and seeking an experienced Hospice Director of Nursing / Director of Clinical Services to lead our clinical team with compassion, accountability, and excellence. This is an excellent opportunity for a hospice RN leader who wants to be more than a number in a large corporate system. Our leadership team is hands-on, accessible, and committed to supporting our nurses, aides, patients, and families. The Director of Nursing is responsible for the overall direction of hospice clinical services, including quality of care, clinical supervision, regulatory compliance, QAPI, chart audits, IDG participation, staff development, and patient care oversight. These duties align with your attached Director of Clinical Services job description, which includes responsibility for the direction of clinical services, maintaining quality patient care, supervising clinical staff, performing chart audits, participating in QAPI, and ensuring Medicare, state, and CHAP compliance.

Requirements

  • Graduate of an accredited School of Nursing
  • Current Ohio Registered Nurse license in good standing
  • Minimum of 2 years of nursing experience in hospice, home health, or skilled home care
  • Hospice experience strongly preferred
  • Minimum of 1 year of supervisory or clinical leadership experience preferred
  • Knowledge of Medicare hospice regulations, Ohio state requirements, CHAP standards, QAPI, and IDG process preferred
  • Strong leadership, communication, organization, and documentation skills
  • Ability to travel to patient homes and facilities throughout the service area
  • Valid driver’s license, reliable transportation, and auto insurance

Responsibilities

  • Lead, supervise, and support hospice clinical staff, including RNs, LPNs, STNNAs, and interdisciplinary team members.
  • Oversee direct and indirect patient care services.
  • Ensure hospice care is delivered according to the plan of care, Medicare Conditions of Participation, Ohio regulations, CHAP standards, and agency policies.
  • Participate in and help lead IDG meetings.
  • Perform ongoing chart audits and ensure documentation supports compliance, quality care, and reimbursement.
  • Gather and review data for QAPI and participate in performance improvement activities.
  • Support referral review, intake review, and appropriateness of hospice services.
  • Assist with clinical staff orientation, education, in-services, coaching, and annual performance evaluations.
  • Provide leadership guidance to nursing and aide staff to maintain quality and consistency of care.
  • Oversee maintenance of accurate clinical records.
  • Conduct patient visits in homes, assisted living facilities, and skilled nursing facilities as needed.
  • Promote a positive, accountable, team-centered clinical culture.
  • Work collaboratively with the Administrator, Medical Director, IDG team, and office leadership.
