About The Position

The Hospice Coordinator is directly responsible for the preparation of paperwork for the Interdisciplinary Team or Group, with special attention to Certification of Terminal Illness information given to the Medical Director for review and collection of signatures for all Hospice orders and IDT paperwork from IDT members. The coordinator oversees all functions of the office to be sure that the Hospice is in compliance for all regulations regarding CTI, IDT, and orders. The Hospice Coordinator functions as a Liaison between patients and staff when patients or families call to the office. Confidentiality and HIPAA are adhered to at all times. Prepares all paperwork for IDT and insures that all is turned in appropriately and timely by IDT members. Makes sure that all forms are signed within the appropriate time frame set forth by Medicare and Medicaid regulations. Audits for all plan of care orders are completed timely and prepared for Clinical Directors final audit so that they can be sent to MD timely. Answers in-coming calls and directs them to the appropriate party and/or answers questions when necessary. Oversees charts and filing to ensure volunteers and file clerks are keeping up with processing paperwork timely and accurately. This also includes breaking down recent discharges and deaths and preparing them for bereavement. Maintains minutes for all Hospice meetings and in-services. Oversee attestations on death certificates to be sure they are sent to the funeral homes appropriately and the Medical Director has either signed or called attestation. Communicates with the exchange regarding schedule and schedule changes for “On Call”.

Requirements

  • High School Diploma
  • Typing 40 WPM
  • Working Knowledge of Excel and Word
  • Excellent time management skills

Nice To Haves

  • AA or AS degree
  • 3 years Hospice or Home Health Administrative Assistant experience
