About The Position

The Hospice Community Liaison (HCC) role is a field-based position responsible for building relationships with referral sources and representing Moments Hospice. The company is seeking experienced hospice sales representatives, healthcare account executives, or clinicians who are passionate about end-of-life care and possess strong relationship-building skills. The role emphasizes compassion, connection, and follow-through. Moments Hospice is a rapidly growing provider known for its patient- and family-centered approach, referred to as 'The Moments Way'. Requirements

Ability to build real relationships.
Drive, empathy, and follow-through.
Experience in hospice or home health sales.
Experience in medical device or pharma sales.
Experience as a clinical liaison or RN liaison.
Experience in SNF or AL business development.
Outside sales experience from other industries.
Clinicians (RN, social work, chaplaincy) ready to step into sales.
Anyone ready to step into hospice sales with the right drive and heart.
