About The Position

The Hospice Community Liaison (HCC) role is a field-based position responsible for building relationships with referral sources and representing Moments Hospice. The company is seeking experienced hospice sales representatives, healthcare account executives, or clinicians who are passionate about end-of-life care. The role emphasizes compassion, connection, and follow-through. Requirements

  • Ability to build real relationships.
  • Drive, empathy, and follow-through.
  • Experience in hospice or home health sales.
  • Experience in medical device or pharma sales.
  • Experience as a clinical liaison or RN liaison.
  • Nursing, social work, or chaplaincy background with a desire to move into sales.
  • Experience in SNF or AL business development.
  • Outside sales experience from other industries.
  • Readiness to step into hospice sales with the right drive and heart.

Responsibilities

  • Build and maintain relationships with referral sources.
  • Represent Moments Hospice to potential referral partners.
  • Connect with patients and families.
  • Follow through on commitments to referral sources and patients.
  • Collaborate with clinical and operations teams to ensure promised services are delivered.
