About The Position

Lower Cape Fear LifeCare is an award-winning nonprofit hospice and palliative care provider serving families across southeastern North Carolina and northeastern South Carolina for over 40 years. They are seeking a passionate Hospice CNA to join their team. The role involves providing direct patient care in various home settings, including personal residences, assisted living facilities, and skilled nursing facilities. The CNA will work closely with RN case managers, train caregivers, and manage supplies. Requirements

  • High school diploma or equivalent.
  • Completion of CNA course.
  • Must attend 12 hours of in-service training supervised by a Registered Nurse on a rolling 12-month basis.
  • Competency of the CNA is assessed in the home every 14 days (or annually unless there is an area of concern).
  • Has not had a lapse of 24 consecutive months where the aide has not provided services for compensation.
  • Current certification with State Board of Nursing, or current registration with Department of Facilities Service in the State of North Carolina/ South Carolina.

Nice To Haves

  • Two years experience with hospice, home health or oncology preferred.

Responsibilities

  • Provide direct patient care such as bathing, peri-care, dressing, mouth care, assisting to toilet, incontinence care, repositioning in bed, and assisting with meals.
  • Tidy the immediate patient area and change bed linens.
  • Maintain an adequate level of supplies in the vehicle for patient care, picking up new supplies at the branch office.
  • Work closely with RN case managers to communicate patient status changes, as well as patient or family concerns.
  • Train caregivers to provide personal care to their loved one, within the scope of practice.
  • Coordinate visit times with the patient or caregiver for all assigned patients.
  • Chart within the facility EMR if patients are in facilities.
  • Complete documentation at the bedside on a company-provided cellphone.
