Requirements

• Arizona Certified Nursing Assistant (CNA)
• Current CPR certification
• Reliable transportation and valid driver's license
• Strong communication and interpersonal skills
• A compassionate heart and commitment to exceptional patient care

Nice To Haves

• Home health or hospice experience is a plus

Responsibilities

• Provide compassionate personal care in patients' homes
• Assist with daily living activities while promoting dignity and independence
• Observe and communicate patient needs to the hospice nurse
• Encourage comfort, safety, and emotional well-being
• Maintain accurate visit documentation
• Build trusting relationships with patients and families
