About The Position

As the Home and Community Based Services Aide, you will provide support and assistance with personal hygiene and household functions so an individual can remain in their own home. The role serves hospice clients in their homes and includes personal care, daily living assistance, household support, client monitoring and documentation, and maintaining a safe environment. The aide must be able to function in any home situation regardless of the client's age, race, creed, color, sex, disability, or financial condition.

Requirements

Current driver's license, vehicle insurance, and reliable transportation or access to public transportation.
For Pennsylvania Category III and IV: successfully complete one of the following: an agency-developed competency or training program meeting 28 Pa. Code § 611.55; a home health aide training program meeting 42 CFR 484.36; or a training program meeting standards imposed on the agency as a provider in a Medicaid Waiver or other publicly funded program providing PCS services.

Nice To Haves

Current CPR certification or ability to complete it within 90 days of hire.
At least 6 months of home care experience.
Ability to work flexible hours and independently.

Responsibilities

Provide personal care and assist with daily living activities, including bathing, grooming, dressing, ambulation, and medication reminders.
Support household tasks and meal preparation, and accompany clients to appointments or errands as needed.
Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use the EVV system.
Ensure a safe environment, operate medical equipment properly, and respond promptly to client needs.
Function in any home situation regardless of the client's age, race, creed, color, sex, disability, or financial condition.
