About The Position

Concho Hearts Hospice is looking for a CNA who can truly make a difference in someone's day. At Concho Hearts Hospice, our CNAs are an important part of the care our patients and families receive. You have the opportunity to spend one-on-one time with patients, build meaningful relationships, and provide the personal care, comfort, and dignity they deserve. We're looking for compassionate and dependable Certified Nursing Assistants (CNAs) / Hospice Aides to join our growing team. We currently have opportunities for both Full-Time and PRN positions. Requirements

Current Texas CNA certification.
Compassionate, dependable, and patient-focused.
Strong communication and interpersonal skills.
Comfortable providing hands-on personal care.
Ability to work independently while being part of a supportive team.
Reliable transportation.
Valid driver's license and current auto insurance.
Ability to travel throughout the assigned service area.
A heart for caring for others.

Responsibilities

Provide personal care including bathing, grooming, dressing, and hygiene assistance.
Assist patients with mobility, transfers, positioning, and other activities of daily living.
Provide care according to the patient's individualized plan of care.
Observe patients and report changes or concerns to the supervising nurse.
Provide companionship, comfort, and emotional support to patients and families.
Help maintain a safe, clean, and comfortable environment for patients.
Complete timely and accurate visit documentation.
Communicate closely with nurses and other members of the hospice care team.
Travel to patients wherever they call home, including private residences and facilities.
