About The Position

As a field-based CNA, you will serve patients in their home or care setting by providing essential personal care services under the supervision of a registered nurse. You will work closely with families, support the care team, and help bring comfort and peace.

Requirements

  • Completion of a state-approved Certified Nursing Assistant (CNA) training program.
  • Active CNA certification in the State of Texas.
  • High school diploma or equivalent required.
  • Compassionate presence and strong communication skills.
  • Basic understanding of hospice care principles and a heart to serve.
  • Ability to read, write, and communicate clearly in English.
  • Comfortable using a smartphone for communication and documentation.
  • Valid Texas driver’s license, reliable transportation, and current auto liability insurance.

Nice To Haves

  • At least one year of experience in hospice, home health, or a similar care setting is preferred.

Responsibilities

  • Providing direct personal care under the supervision of the RN, in alignment with the CNA Plan of Care and patient preferences.
  • Helping patients transfer, move, and ambulate safely — while maintaining a calm, clean, and secure environment.
  • Noticing and promptly reporting any changes in the patient’s condition or behavior to the RN, supporting proactive care.
  • Following all safety and hygiene protocols to protect the health of patients, families, and teammates.
  • Upholding patient confidentiality, respecting personal property, and communicating with kindness and professionalism.
