About The Position

Transitions Hospice is hiring a Certified Nursing Assistant to provide care to patients in their homes across Champaign, Piatt, and surrounding areas. CNAs are vital members of the interdisciplinary care team, providing unhurried, dignified care and serving as a trusted presence for patients and families. The organization provides 24/7 support addressing patients’ and families’ physical, emotional, and spiritual needs. The schedule is Monday through Friday, with 8-hour shifts. The listed pay range is $19.00–$24.00 an hour. This role involves local travel for patient visits.

Requirements

Active CNA license or certification in the state of employment.
Current CPR certification.
Comfort and sensitivity working with terminally ill patients and their families.
A calm, dependable presence under emotionally demanding circumstances.
Strong attention to detail in observation and documentation.
Valid driver's license and willingness to travel locally for patient visits.

Nice To Haves

One or more years of experience in hospice, home health, or a related organization is preferred.

Responsibilities

Provide personal care with dignity, comfort, and respect.
Attend to patient needs and requests promptly and compassionately.
Monitor and record vital signs, including temperature, pulse, respiration, and blood pressure, as directed.
Observe patients and report changes to the care team accurately and promptly.
Communicate with patients and families in a positive, ethical, and confidential manner.
Maintain thorough documentation in line with organizational standards.
Uphold the hospice philosophy with professionalism and genuine compassion.
