About The Position

Transitions Hospice is seeking a Certified Nursing Assistant to provide compassionate, dignified care to hospice patients in their homes across Peoria and surrounding areas. The CNA is a vital member of the interdisciplinary care team and a trusted presence for patients and families. The role supports patients’ physical, emotional, and spiritual needs through personal care, attentive observation, clear communication, and thorough documentation. The schedule is Monday through Friday, eight-hour shifts, with an on-call rotation.

Requirements

Hold an active CNA license or certification in the state of employment.
Possess and maintain current CPR certification.
Be comfortable and sensitive when working with terminally ill patients and their families.
Maintain a calm, dependable presence under emotionally demanding circumstances.
Demonstrate strong attention to detail in observation and documentation.
Have a valid driver's license and willingness to travel locally for patient visits.

Nice To Haves

One or more years of experience in hospice, home health, or a related organization.

Responsibilities

Provide personal care with dignity, comfort, and respect.
Attend to patient needs and requests promptly and compassionately.
Monitor and record vital signs, including temperature, pulse, respiration, and blood pressure, as directed.
Observe patients and report changes to the care team accurately and promptly.
Communicate with patients and families in a positive, ethical, and confidential manner.
Maintain thorough documentation in accordance with organizational standards.
Uphold the hospice philosophy with professionalism and genuine compassion.
