About The Position

Coordinate and oversee direct and indirect patient services provided by clinical personnel. provides supervision and guidance to clinical staff to ensure the quality of services and maintenance of standards. Perform Chart Audits Appraise clinical staff performance during their probationary period and annually, provide teaching and coaching necessary to enhance the delivery of services and the professional development of nursing and health aide staff. Identify and evaluate patient needs and appropriateness of home care by reviewing the initial intake screening form before assignment of direct staff. Assist with the orientation of new staff as well as in-services for current staff. Visit patients in residential locations as needed. Maintains adequate and appropriate medical supplies. Responsible for ensuring Medicare, state, and CHAP regulations are followed by clinical staff. Assist the Director of Clinical Services with responsibilities as requested. Assume responsibility for self-development by continually striving to improve his/her Nursing/management practice through formal education, attendance to workshops and conferences, and active participation in professional and related organizational meetings. Travel to and from patients' residences when supervision of care is scheduled.

Requirements

  • Licensed Registered Nurse
  • A minimum of 2 years of Hospice leadership experience REQUIRED
  • Valid Drivers License and Auto insurance Coverage
  • Knowledge of business management, governmental regulations, and CHAP standards
  • Use of HCHB experience preferred
