About The Position

Presbyterian Homes & Services – Optage Hospice is seeking an experienced Hospice RN Case Manager looking to use their knowledge and skills in a leadership position as a Hospice Clinical Supervisor. This position is ideal for those who feel called to hospice and wish to serve older adults within a caring Christian environment. Requirements

  • Registered nurse with current licensure with the State Board of Nursing in state(s) in which he/she practices.
  • Three to five (3 - 5) years of experience in Home Care/Hospice.
  • Demonstrated competencies in home care/hospice care management and operations, quality and business management, team building and employee development, client and family support systems, governmental regulations and community outreach.
  • Demonstrated knowledge and experience in home care or hospice and palliative care clinical practices.
  • Must have a valid driver’s license and proof of auto insurance.
  • Demonstrated computer proficiency including Microsoft Office and hospice software programs.
  • Demonstrated compatibility with PHS's mission and operating philosophies.
  • Demonstrated ability to read, write, speak and understand the English language to communicate with all customers.

  • BSN or Masters Degree preferred.

Responsibilities

  • Establish and maintain communication systems that support good customer relations. Collaborate with management and site leaders to ensure seamless access to services in assigned clinical area.
  • Oversee the implementation of client care processes: Selection, assessment, planning, implementation, and evaluation of nursing care based upon regulations and established clinical best practices.
  • Ensure that all services provided promote dying as part of the normal process of living, focusing on maintaining the quality of remaining life. Participate in client care processes and support case managers with caseload when necessary. Lead and participate in IDG meeting and oversee compliance of clinical charting.
  • Coordinate clinical services with other health professionals inside and outside the organization that promotes a spirit of cooperation and maintain effective working relationships to establish effective benchmarking opportunities and clinical best practices.
  • Provide ongoing employee educational and development opportunities which meet regulatory requirements and opportunities for growth.
  • Assist in recruiting, selecting, hiring and orienting new team members.
  • Responsible for building and maintaining employee morale and motivating employees to reach their full potential in providing excellent end of life care.
  • Participate in the development, training and implementation of clinical policies, procedures and practices that are consistent with regulations and established clinical best practices.
  • Oversees the maintenance of patient clinical records for purposes of evaluation and reporting of organizational activities and regulatory compliance.
  • Manage resource allocations in order to achieve or exceed operational goals and demonstrate good stewardship of available resources.
