The Hospice Clinical Supervisor is responsible for supporting and coordinating day-to-day clinical operations in collaboration with the Hospice Clinical Director to ensure timely delivery of patient care and to improve clinical/quality outcomes, patient satisfaction and financial sustainability. The Assistant Manager also functions as a clinical resource and mentor for the Hospice nursing staff, as well as for the entire interdisciplinary team.

Requirements

  • Minimum Education: Graduate of an accredited school of nursing required. BSN preferred.
  • Minimum Work Experience: 1-3 years or more of clinical experience in a healthcare setting; preferably Hospice or Home Health.
  • Required Licenses/ Certifications/Background Checks: Valid Vermont Driver’s License and possess a current nursing license in Vermont. Ability to successfully pass pre-employment driver record, criminal, adult and child abuse background, and Office of Inspector General (OIG) check/review.

Responsibilities

  • Provide day-to-day clinical oversight and coordination of hospice services, ensuring safe, timely, and patient-centered care delivery in partnership with the Hospice Clinical Director.
  • Serve as a clinical leader and mentor to nursing staff and the interdisciplinary team, offering guidance on patient care, documentation standards, and evidence-based best practices.
  • Apply independent clinical judgment to prioritize patient needs, resolve care delivery challenges, and drive quality outcomes, patient satisfaction, and financial sustainability.
  • Work effectively within interdisciplinary team.
  • Excellent communication skills (person-to-person, telephone, written, verbal, group facilitation).
