About The Position

Puget Sound Home Health and Hospice is seeking an experienced and caring full-time Hospice Clinical Manager (RN) for our Hospice division in the Pierce County area! This role ensures agency compliance with all state/federal hospice laws & regulations, reviews incoming referrals for appropriateness and manages approvals & assignments, and reviews patient charts for medical diagnoses, procedures, medications, & clinical course. The manager also assists the DCS with planning, implementing & evaluating in-services & CEU programs, assists with interviewing, hiring, training, & mentoring organization personnel, and participates in QAPI, OASIS review, and coding workflow. Additionally, the role involves providing symptom management and comfort care for hospice patients in their home and taking call when needed.

Requirements

  • A minimum of 1 year of health care management experience, preferably hospice
  • Must have at least 1 year of hospice agency experience, including admissions
  • Must be experienced in HOPE charting requirements for hospice
  • Current BLS certification
  • Current Washington state Registered Nurse (RN) license
  • Experience with Home Care Home Base preferred, but not required
  • Knowledge of hospice regulations, including Medicare COPs, PDGM, VBP
  • Licensed and insured driver with reliable transportation

Nice To Haves

  • A BSN degree is preferred, but not required
