About The Position

The Hospice Clinical Liaison (RN) is responsible for maintaining relationships with referral sources, including hospitals, long-term care facilities, rehabilitation centers, and physician practices. This role works closely with agency sales representatives to ensure effective communication and smooth transition of referrals. The liaison will efficiently process referrals according to agency and referring provider policies, and accurately explain Eden Health's program to patients, family members, and other referral sources. They will collaborate with agency staff to ensure a safe transition for patients to home, provide timely documentation of marketing activities, and follow up on referrals for evaluation or admission. The position also involves attending in-services, staff meetings, and case conferences, completing all documentation accurately and legibly, and obtaining outstanding orders or documentation as requested. The liaison must be able to communicate effectively with patients, coworkers, and other healthcare professionals, participate in process development and implementation, and meet or exceed home health and hospice referral and admission goals.

Requirements

  • High school diploma or equivalent required
  • General clerical, data entry and excellent organizational skills
  • Able to communicate effectively in English verbally and in writing
  • Working knowledge of computer software programs
  • Valid driver’s license and reliable transportation
  • Able to pass criminal background check and national sex offender clearance
  • Must be able to physically navigate through community for entire shift. This is not an in-office administrative sales position.

Nice To Haves

  • Clinical background preferred
  • Healthcare marketing experience in home health and/or hospice industry, DME, medical supplies outside sales, pharmaceutical sales, and/or other industries which may call on contacts at SNF, Hospitals, Dr. Offices, ALFs and more
  • B.S. Degree in Marketing, Sales, Business or Health Care preferred

Responsibilities

  • Responsible for maintaining relationships with referral sources including, but not limited to hospitals, long term care facilities, rehabilitation centers, and physician practices.

  • Explains Eden Health’s program to patients, family members, and other referral sources in an accurate, complete, and thorough manner.
  • Works closely with agency staff to guarantee all patients experience a safe transition to home.
  • Provides accurate and timely documentation of all marketing activities back to the agency as requested.
  • Follows-up on referrals for evaluation and/or admission to Eden Health program.
  • Attends and participates in in-services, staff meetings, and case conferences.
  • Completes all documentation in an accurate, timely, and legible manner.
  • Obtains outstanding orders or documentation as requested by agency.
  • Ability to communicate effectively with patients, coworkers, and other healthcare professionals.
  • Participate in the development and implementation of processes in accordance with agency’s evolving goals.
  • Meet and/or exceed home health and hospice referral and admission goals.
