About The Position

Advantage Hospice is growing and seeking a relationship-builder with knowledge of the Northeast Ohio hospice market to own a territory. This role is ideal for someone who enjoys being in the field, initiating conversations, and building lasting referral partnerships. It is a hospice-specific liaison/community care consultant position focused on relationship building and diligent follow-up.

Requirements

• 1–3 years in healthcare marketing, community outreach, or a liaison role (hospice, palliative care, or senior care preferred).
• Existing relationships with referral sources in Northeast Ohio.
• Knowledge of hospice and the referral process.
• A self-starter who's organized, outgoing, and professional.

Responsibilities

• Strengthen existing referral relationships and develop new ones.
• Make calls, schedule meetings, present services in person, and follow up diligently.
• Identify, target, and develop new referral partners.
• Represent Advantage Hospice at community events.
