About The Position

Acts Hospice is currently seeking qualified candidates for our Hospice Clinical Director position. In this role, you will be responsible for coordinating the planning, implementation, and delivery of high-quality, compassionate, and cost-effective hospice care. Evaluates the effectiveness of hospice services and ensures the timely implementation, coordination, and ongoing evaluation of individualized patient care plans. Advocates for the physical, emotional, psychosocial, and spiritual needs of patients and their families while promoting dignity, comfort, and quality of life throughout the end-of-life journey. Serve as a knowledgeable resource, advocate, and educator to patients, families, caregivers, and members of the healthcare team, supporting best practices and continuous improvement in the care of individuals with life-limiting and terminal illnesses.

Requirements

  • Bachelor of Science in Nursing (BSN) degree, either completed or currently in pursuit
  • Minimum of 2-3 years of professional nursing experience
  • Experience in hospice, home health, geriatrics, oncology, or a comparable clinical setting is strongly preferred
  • Active Registered Nurse (RN) license in good standing with the applicable state Board of Nursing
  • Current CPR certification (or obtain within three months of employment)
  • Must possess and maintain a valid state driver’s license

  • Experience in hospice, home health, geriatrics, oncology, or a comparable clinical setting
