Due to our continued growth, we are adding a Hospice Clinical Coordinator Assistant for our Fort Wayne Office.

Requirements

• Experience in medical records and working in an office setting
• Experience in customer service
• Current driver's license
• Must have reliable transportation and insurance

Nice To Haves

• One year of Hospice experience preferred, but not necessary
• CNA License is preferred, but not necessary
• Preferred Licensed Certified Nursing Assistant in State of Indiana

Responsibilities

• Manage Hospice Medical Records blast for your team and follow up on all requests in a timely manner including providing feedback to team member when requests are completed.
• Complete all routine plotting duties and incoming plotting requests as needed and in a timely manner.
• Manage all DME orders, pick-ups and follow-up requests as needed in a timely manner.
• Call funeral homes daily on any previous day patient deaths and follow death certificate physician verification process.
• Order x-rays & labs as requested.
• Answer large amounts of incoming calls from patients, families and referral sources and show professionalism and competency in assisting and/or directing calls to the appropriate department.
