Hospice Chaplain
About The Position

Our Hospice Chaplains offer more than prayers and presence; they bring comfort, guidance, and meaning when it’s needed most. We’re looking for a compassionate Hospice Chaplain to provide non-denominational spiritual and emotional support to patients and families facing end-of-life care. As a valued member of our interdisciplinary team, you’ll help ensure that no one walks this path alone. As a Hospice Chaplain, you will provide spiritual and emotional support to patients, families, and caregivers in alignment with their beliefs and values, offering non-denominational, inclusive care that respects all faiths, spiritual traditions, and cultural backgrounds. You will serve as an integral member of the interdisciplinary team, participating in patient care conferences and collaborating with nurses, social workers, aides, and medical staff. Your role includes assessing spiritual needs, developing individualized spiritual care plans, conducting visits, prayers, blessings, and rituals as appropriate, assisting families during grief and loss, providing bereavement support, and building relationships with local faith leaders and community resources. You will also maintain timely and accurate documentation of spiritual assessments, interventions, and outcomes.
Requirements
- Bachelor’s degree in theology, religion, human services, counseling, psychology, or sociology (or as required by state-specific regulations)
- A minimum of one unit of ACPE-accredited Clinical Pastoral Education (CPE)

Responsibilities
- Provide spiritual and emotional support to patients, families, and caregivers in alignment with their beliefs and values.

- Build relationships with local faith leaders and community resources to support patients' and families' unique spiritual needs.
- Maintain timely and accurate documentation of spiritual assessments, interventions, and outcomes.
