About The Position

This is a part-time Hospice Chaplain position focusing on providing spiritual care. The role involves assessing patients' spiritual needs, developing care plans, offering spiritual support, and collaborating with a multidisciplinary team to ensure holistic patient care. The chaplain will also assist with bereavement services, support families through grief and end-of-life transitions, and document patient interactions. Additionally, the position requires maintaining compliance with Hospice regulations and providing support to Hospice staff.

Requirements

  • Bachelor's Degree
  • One year of related experience.
  • Working knowledge of standard practices and procedures.
  • Driver's License
  • ACLS Certification
  • Strong interpersonal and communication skills.
  • Ability to work with individuals of diverse backgrounds and belief systems.
  • Compassionate presence and active listening skills.
  • Ability to manage emotional and sensitive situations professionally.
  • Strong organizational and documentation skills.

  • Board Certified Chaplain through a recognized organization
  • Ecclesiastical endorsement or ordination

Responsibilities

  • Provide spiritual care assessments and develop individualized spiritual care plans.
  • Offer counseling, prayer, rituals, and spiritual support based on patient family preferences.
  • Participate in IDT meetings and contribute to the ongoing plan of care. Communicate relevant spiritual concerns to the team members.
  • Collaborate with nurses, physicians, social workers, aides, and volunteers to ensure holistic patient care.
  • Assist in bereavement follow-up and grief support services as appropriate and provide guidance to families before and after death.
  • Support patients and families coping with grief, loss, and end-of-life transitions.
  • Respect and honor all cultural, spiritual, and religious beliefs and practices.
  • Facilitate connections with community clergy or spiritual resources when requested.
  • Document all patient interactions in the electronic medical records in a timely manner.
  • Maintain compliance with Hospice regulations, policies, and standards.
  • Provide spiritual support to Hospice staff as needed (e.g., debriefing after patient death).
  • Participate in community outreach or education related to Hospice services when applicable.
