About The Position

This is a part-time Hospice Chaplain position focusing on providing spiritual care. The role involves assessing patients' spiritual needs, developing care plans, offering spiritual support, and collaborating with a multidisciplinary team to ensure holistic patient care. The chaplain will also assist with bereavement services, support families through grief and end-of-life transitions, and document patient interactions. Additionally, the position requires maintaining compliance with Hospice regulations and providing support to Hospice staff.

Requirements

Bachelor's Degree
One year of related experience.
Working knowledge of standard practices and procedures.
Driver's License
ACLS Certification
Strong interpersonal and communication skills.
Ability to work with individuals of diverse backgrounds and belief systems.
Compassionate presence and active listening skills.
Ability to manage emotional and sensitive situations professionally.
Strong organizational and documentation skills.

Responsibilities

Provide spiritual care assessments and develop individualized spiritual care plans.
Offer counseling, prayer, rituals, and spiritual support based on patient family preferences.
Participate in IDT meetings and contribute to the ongoing plan of care. Communicate relevant spiritual concerns to the team members.
Collaborate with nurses, physicians, social workers, aides, and volunteers to ensure holistic patient care.
Assist in bereavement follow-up and grief support services as appropriate and provide guidance to families before and after death.
Support patients and families coping with grief, loss, and end-of-life transitions.
Respect and honor all cultural, spiritual, and religious beliefs and practices.
Facilitate connections with community clergy or spiritual resources when requested.
Document all patient interactions in the electronic medical records in a timely manner.
Maintain compliance with Hospice regulations, policies, and standards.
Provide spiritual support to Hospice staff as needed (e.g., debriefing after patient death).
Participate in community outreach or education related to Hospice services when applicable.
