About The Position

Advocate Health at Home provides in-home hospice care focusing on comfort, pain management, and emotional and spiritual support. Professionals provide compassionate hospice care for patients and their families in the comfort of home. Requirements

  • At least Two (2) units of CPE, accredited by ACPE; NACC units may be considered. 5 units preferred.
  • In good standing with continued endorsement by the appropriate authority of his/her church, faith group or religious institute
  • A valid driver’s license issued by the Division of Motor Vehicles.
  • Completed masters degree (MA, MDiv) in religious studies, theology, divinity or related field.
  • A strong background in functional theology and the ability to apply this in a pastoral manner to individuals and families.
  • Excellent communication skills with knowledge of counseling techniques.
  • Ability to identify clients’ needs with consideration to cultural, ethnic, religious, economic, and other diverse backgrounds
  • Experience and compassion in ministering to those who are dying and their grieving families.

Nice To Haves

  • 5 units of CPE preferred.

Responsibilities

  • Provide comprehensive holistic spiritual care to patients, families, and healthcare staff, including counseling, crisis response, and sacramental support.
  • Assist during emergencies, guides families through illness and end‑of‑life processes and promote patient safety.
  • Collaborate with interdisciplinary teams, participate in care planning, education, and ethical discussions, and supports staff through consultations and training.
  • Contributes to departmental operations, mentoring, and community outreach while upholding organizational values and maintaining active involvement in their faith community.
  • Participates in the QA/CQI process for Advocate Hospice.
