Requirements

  • Comfortable in an interfaith setting.
  • Able to accept different lifestyles, cultures, beliefs, and values.
  • Must be a licensed driver with an insured automobile.
  • The ability to communicate well, both verbally and in writing.
  • Physical activities will include walking, sitting, stooping, and standing.
  • Ordained, commissioned or credentialed according to the practices of an organized religious group, and has completed one unit of CPE or has a minimum of a bachelor’s degree with emphasis in counseling or related subjects and has within 90 days of hire, completed specific training to include: common spiritual issues in death and dying, belief systems of comparative religions related to death and dying, spiritual assessment skills, individualizing care to patient beliefs, and varied spiritual practices/rituals OR Has a degree in theology or from a seminary along with emphasis in counseling or related subjects and has within 90 days of hire, completed specific training to include: common spiritual assessment skills, individualizing care to patient beliefs, and varied spiritual practices/rituals.
  • Be able to demonstrate proficiency in Internet browsers (e.g., Explorer, Chrome, and Fire Fox), Microsoft Office programs including Outlook, Excel, Word and Publisher, electronic health records (EHR) systems, and any HR related systems if applicable.

  • Hospice experience preferred.

Responsibilities

  • Conduct a spiritual assessment as part of the comprehensive admission assessment.
  • Assist in the development and implementation of the interdisciplinary patient care plan as it pertains to spiritual care.
  • Provide spiritual support in accordance with the ongoing wishes and needs of the patient/family, respecting their religious preferences or definition of spirituality.
  • Work in close collaboration with local clergy when desired by the patient.
  • Maintain and submit written clinical records per agency policy, including the initial evaluation, the care plan, and daily notes.
  • Evaluate the patients' and family's response to and effectiveness of the spiritual counseling.
  • Confirm the scheduling of visits with patients on a weekly basis, coordinating with Case Managers to facilitate other staff visits.
  • Participate in IDG and QUPI meetings and activities.
  • Conduct religious services of prayer, worship, and rituals for patients/families, as appropriate.
  • Collaborate with other professionals in resolving spiritual/ethical issues.
  • Provide spiritual and bereavement support to staff as needed.
  • Provide consultation and education to the IDG on spiritual care.
  • Provide funeral or memorial services for patients as requested.
  • Assume responsibility for self-development by continually striving to improve one's spiritual practice through formal education, attendance at workshops and conferences, active participation in professional and related organizational meetings, and/or individual research and reading.
  • Other duties as assigned.
