The Hospice Chaplain is responsible for the provision of spiritual care services to patients, family members and caregivers of Hospice Care Options, either directly or through coordination of care with other spiritual caregivers. The Hospice Chaplain is responsible for spiritual assessment of all patients and the development of an Individualized Plan of Care based upon the assessment. The Chaplain functions as an integral member of an Interdisciplinary Team in the provision of spiritual care, working with other disciplines to provide integrated, holistic end of life care. The Chaplain will also be responsible for the facilitation of funeral services when requested by family members. Bereavement Care and Grief Support are also elements of the work of the Hospice Chaplain, as well as the planning and facilitation of annual ecumenical Memorial Services held in memory of deceased patients.
