About The Position

Newport Hospice in Irvine, CA is seeking a compassionate and dedicated Hospice Chaplain / Bereavement Counselor to join our team on a per diem basis. In this meaningful role, you will provide spiritual care and emotional support to patients and their families during life's most profound moments. This position serves as an active member of the hospice interdisciplinary team and works collaboratively with physicians, nurses, social workers, hospice aides, volunteers, and other members of the care team to address spiritual, emotional, cultural, and bereavement needs. The Chaplain provides support consistent with each patient's and family's individual beliefs, values, cultural background, and preferences. Requirements

  • Education, training, or demonstrated experience in spiritual/pastoral care appropriate to hospice patients and families.
  • Knowledge of death, dying, grief, bereavement, cultural humility, and spiritual assessment.
  • Ability to provide spiritual care consistent with the patient's individual beliefs, including patients who identify as religious, spiritual but not religious, agnostic, or atheist.
  • Ability to participate as a member of the hospice interdisciplinary group and contribute to the plan of care.
  • Hospice, palliative-care, healthcare, or pastoral-care experience a must.
  • Reliable transportation and valid CA driver's license to travel to patient care locations.

Nice To Haves

  • Bachelor's degree in theology, religious studies, ministry, pastoral care, counseling, psychology, social sciences, or a related field is preferred.

Responsibilities

  • Complete spiritual assessments and identify patient and family needs.
  • Provide spiritual counseling, emotional support, prayer, life review, and supportive presence based on patient preferences.
  • Develop individualized spiritual goals and interventions for the hospice plan of care.
  • Provide anticipatory grief and bereavement counseling to patients and families.
  • Complete bereavement assessments and Plans of Care (POC) and follow-up with families after a patient's death.
  • Identify individuals who may need additional grief or mental health support and provide appropriate referrals.
  • Participate in interdisciplinary group (IDG) meetings and collaborate with nursing, social work, physicians, and other hospice team members.
  • Coordinate with clergy and community spiritual resources when requested.
  • Assist with memorial services, grief support activities, and bereavement programs.
  • Complete accurate and timely clinical documentation.
  • Respect all religious, spiritual, cultural, and nonreligious beliefs and preferences.
