Requirements

• Compassionate and committed to improving patients’ quality of life
• Ability to work collaboratively with Nurses, Social Workers, Chaplains, Volunteers, and Aide Leadership
• Comfortable working one-on-one with patients while being supported by a dedicated care team
• Able to work independently and manage daily responsibilities
• Willingness to travel with reliable transportation, a valid driver’s license, and current auto insurance
• Current CNA certification in the state(s) of practice

Responsibilities

• Works under the direction of the Nursing Leadership Team to provide compassionate care to assigned hospice patients in accordance with their assessments and individualized care plans.
• Collaborates with the hospice care team to meet patient needs, promote comfort and dignity, and report changes in condition.
• Follows all Company policies, procedures, safety guidelines, and infection-control practices.
