About The Position

Residential Home Health and Hospice is seeking a Certified Nursing Assistant for a Hospice position. This role involves providing care to patients in their homes in the Milford, PA area. The position emphasizes compassionate, patient-centered care within an interdisciplinary team focused on comfort, dignity, and quality of life. Responsibilities include assisting patients with activities of daily living, observing and reporting changes in condition, supporting patients and caregivers, and documenting care in the EMR. The role requires travel to patient homes within an assigned territory.

Requirements

• Active Certified Nursing Assistant (CNA) certificate in the state you work
• High School Diploma or certificate
• Minimum one year healthcare experience
• Valid driver’s license
• Ability to spend approximately 20% of your workday driving to and from patient locations

• Hospice experience preferred

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
• Provide hands-on care to patients, assisting with activities of daily living (ADLs) while observing and reporting changes in condition to support comfort and quality of life
• Support and guide patients and caregivers by reinforcing the care plan, offering emotional support, and helping them understand what to expect during the hospice journey
• Respond promptly to patient needs with empathy and attentiveness and accurately document care provided in the EMR
• Travel to patient homes within assigned territory
