About The Position

Residential Home Health and Hospice is a nationally recognized leader in home health and hospice, and a consistent Top Workplace. We are hiring a Certified Nursing Assistant for a Hospice position providing care to patients in their homes in the Elgin, IL area and is eligible for a $5,000 Sign-on Bonus. This role involves delivering compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life. The position requires providing hands-on care to patients, assisting with activities of daily living (ADLs) while observing and reporting changes in condition to support comfort and quality of life. It also involves supporting and guiding patients and caregivers by reinforcing the care plan, offering emotional support, and helping them understand what to expect during the hospice journey. The role requires responding promptly to patient needs with empathy and attentiveness and accurately documenting care provided in the EMR. The position allows for independent work while traveling to patient homes within an assigned territory.

Requirements

• Active Certified Nursing Assistant (CNA) certificate in the state you work
• High School Diploma or certificate
• Minimum one year healthcare experience
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
• Legally authorized to work in the United States

Responsibilities
