About The Position

Optimal Care is seeking a dedicated Hospice Certified Nursing Assistant (CNA) to provide comfort, dignity, and compassionate care to patients during life's most meaningful moments. As a Hospice Aide/Homemaker, you will offer hands-on support to help patients maintain their quality of life while remaining in their homes. This involves assisting with personal care, including bathing, dressing, grooming, toileting, oral hygiene, meal preparation, and mobility. The role is designed for a caring professional who finds purpose in building meaningful connections and serving others with empathy and compassion. You will develop trusting relationships with patients and families while collaborating closely with nurses and the interdisciplinary care team to support each patient's unique needs. Every visit is an opportunity to provide reassurance, preserve dignity, and make a lasting impact during an important stage of the patient's journey. This is a high travel position requiring daily travel within the service area to provide care to patients in their homes, assisted living facilities, or nursing homes.

Requirements

  • Completed a minimum of 75 hours of classroom and supervised practical training.
  • Sixteen hours of classroom must be completed prior to a mandatory 16 hours of practical training.
  • Must hold current certification as a nurse aide and be in good standing on the state nurse aide registry.
  • Reliable transportation, valid driver’s license, and valid automobile insurance coverage.
  • Must be able to read and write in English and follow written and verbal instructions in English effectively.
  • Understands hospice philosophy, comfortable providing specialized care to the terminally ill in the home or facility.
  • Is self-directing with the ability to work with little direct supervision.
  • Secure with issues of death/dying.
  • Provides a calm manner when in a patient’s home.
  • Has empathy for the needs of the ill, injured, frail and the impaired.

Responsibilities
