About The Position

At Suncrest Hospice, our goal is to change the expectation of hospice care by providing exceptional care and service to our patients. This is achieved by allocating resources to increase staff to patient ratios, thereby increasing clinical visits while lowering clinician caseloads. We are proud to be a Community Health Accreditation Partner (CHAP) certified hospice. Requirements

• Must have a reliable personal vehicle for daily travel to assigned facilities and patients within Suncrest's service territory.
• Must be able and willing to commute daily throughout Linn County and surrounding counties.
• Ability to work well within a team environment.
• Able to work independently and on your own from a day-to-day basis.
• Must be willing to drive with reliable transportation, valid driver’s license, and auto insurance.
• Certified Nursing Assistant (CNA) certification in the state(s) of practice.

Responsibilities

• Providing resident care to assigned residents in accordance with resident assessments, care plans and as directed by nursing management.
• Working with other licensed care staff to coordinate the proper types of care to residents.
• Strictly following all Company nursing policies and procedures as well as safety guidelines.
• Driving daily to assigned facilities and patients within Suncrest's service territory.
• Working closely with patients on a 1-1 level.
• Collaborating daily with nurses, social workers, chaplains, volunteers, and aide leadership.
